Fusarium Dimerum Splenic Abscess in an Immunocompetent Individual after Occupational Exposure
Fusarium species are environmental moulds that can cause a variety of infections in both immunocompetent and immunocompromised individuals.A 42-year woman experienced left-upper quadrant pain and shortness of breath for the past four months. Her medical history was notable for intermittent left shoulder tip pain over three years. On examination, splenomegaly was observed along with decreased breath sounds over the left lung base.A contrast-enhanced CT scan revealed a thick-walled collection in the left subphrenic region.Ultrasound-guided aspiration of the pus revealed septate fungal hyphae on wet preparation.Growth of a white-golden cottony mould was obtained on mycological media, which was identified as Fusarium dimerum by an automated microbial identification system. The patient had a history of previous occupational exposure to Fusarium species with a limited use of personal protective equipment (PPE).She was treated with oral voriconazole, and follow-up ultrasound showed no residual fluid in the spleen.
