Decoding the Enigma: Case Series of Secondary Hypertension Presenting as Hemorrhagic Stroke: A Diagnostic Conundrum

Abstract Intracranial hemorrhage (ICH) is a devastating condition with high morbidity and mortality, commonly attributed to poorly controlled hypertension. While primary (essential) hypertension is often the presumed cause, secondary hypertension remains an under-recognized and curable contributor to this condition. Secondary hypertension encompasses a variety of underlying etiologies, including endocrine, renal, and vascular disorders, many of which can have life-altering outcomes if left undiagnosed. This case series highlights three patients who presented with hemorrhagic stroke and were subsequently diagnosed with secondary hypertension. All three patients exhibited features such as refractory hypertension and persistent hypokalemia, which prompted further diagnostic evaluation. Each patient’s underlying condition was identified, leading to targeted medical and surgical interventions that resulted in significant clinical recovery, well-controlled blood pressure, and reduced risk of future complications. These cases underscore the importance of systematically evaluating secondary hypertension in patients with atypical presentations, such as young age, resistant hypertension, or unexplained hypokalemia. Early identification and treatment of secondary hypertension not only improve patient outcomes but also address the root cause, preventing recurrences and long-term complications. This case series highlights the critical need for heightened clinical suspicion and comprehensive diagnostic workups in managing patients with hemorrhagic stroke.
