Aneurysm Wall Permeability Measured by DCE-MRI as a Predictor of Growth in Non-Saccular Intracranial Aneurysms at Follow-up

Motivation: Predicting the rupture risk of non-saccular intracranial aneurysms is critical, as growing aneurysms have higher rupture risk than stable ones. Goal(s): This study aimed to predict the growth of non-saccular intracranial aneurysms using wall permeability (Ktrans) from DCE-MRI. Approach: Patients were classified into growth and non-growth groups based on aneurysm size changes during a median follow-up period of 7.5 months. We investigated the potential of demographic and imaging characteristics in predicting aneurysm growth. Results: Ktrans was the most effective biomarker in identifying aneurysm growth with the highest AUC of 0.778, and higher Ktrans regions were correlated with the direction of aneurysm growth. Impact: This study demonstrates that baseline Ktrans, derived from DCE-MRI, is an effective predictor of non-saccular intracranial aneurysm growth. It offers a valuable tool for predicting aneurysm growth and enhancing rupture risk prediction in clinical practice.
