THE LIPOSARCOMA: UNRAVELING THE MYSTERY OF SOFT TISSUE TUMORS
Liposarcomas (LSs) are rare malignant mesenchymal tumors characterized by adipocytic differentiation. They commonly arise in the lower extremities and retroperitoneum, with welldifferentiated liposarcoma (WDL) being the most prevalent subtype. We present the case of a 65-year-old male with a progressively enlarging, painless swelling in the right gluteal region over ve years, which eventually reached 30 × 16 cm in size. Local examination revealed a soft to rm, bosselated mass with overlying skin changes. MRI ndings suggested a lipomatous neoplasm, and the patient underwent complete surgical excision. Histopathological examination conrmed WDL with no signs of dedifferentiation. Immunohistochemistry was positive for MDM2, supporting the diagnosis. Postoperative recovery was uneventful, and the patient was discharged in stable condition. This case highlights the importance of distinguishing WDL from benign lipomas and more aggressive liposarcoma subtypes. Histopathology and immunohistochemical proling are essential tools in accurate diagnosis and subsequent management. Early surgical intervention with adequate margins remains the cornerstone of treatment, while the roles of chemotherapy and radiotherapy are limited in WDL.
