Necrotising Pneumonia in an Immunocompromised Patient with End-stage Renal Disease: A Case Study
This case study presents a 35-year-old male with end-stage renal disease (ESRD) on maintenance haemodialysis (MHD) who developed necrotising pneumonia following an episode of E. coli sepsis.Despite initial antibiotic treatment for left lobar pneumonia, the patient's symptoms persisted, leading to a diagnosis of necrotising pneumonia.Diagnostic assessments, including HRCT and bronchoscopy with bronchoalveolar lavage (BAL) culture, revealed bilateral patchy consolidation and panfungal growth, respectively. The patient's immunocompromised state due to ESRD likely contributed to the progression of the infection.Treatment included broad-spectrum antibiotics, antifungals, and supportive care, resulting in clinical improvement. The case highlights the increased vulnerability of ESRD patients to severe infections and the importance of early diagnosis and comprehensive treatment.Long-term follow-up is critical due to ongoing risks related to dialysis and potential respiratory complications.
