Costs of home-based respiratory support and oxygen therapy

Objective. To assess the direct costs associated with home-based respiratory support and oxygen therapy (RSOT). Material and methods. The study included two stages: prospective follow-up of patients receiving home-based RSOT and economic analysis of results. There were 98 patients (mean age 61 years, 56.9% men) divided into 3 groups depending on home-based RSOT: invasive mechanical ventilation (IMV) — 8 (8.2%), non-invasive ventilation (NIV) — 43 (43.9%) and long-term oxygen therapy (LTOT) — 47 (47.9%). Direct costs of home-based RSOT were estimated. Results. The weighted average monthly expenses per patient with chronic respiratory failure (CRF) requiring RSOT at home were RUR 115.046 (95% CI: RUR 120.374 — 109.717) for IMV, RUR 39.905 (95% CI: RUR 44.336 — RUR 35.473) for NIV and RUR 10.518 (95% CI: RUR 11.128 — RUR 9.909) for LTOT. Most of these costs were related to provision of necessary equipment. Conclusion. Direct costs for home-based RSOT decrease sequentially from IMV to NIV and LTOT.
