A RARE PRESENTATION OF IDIOPATHIC MULTIPLE THROMBOEMBOLISM IN A MIDDLE-AGED FEMALE: A DIAGNOSTIC AND THERAPEUTIC CHALLENGE
Background: Idiopathic venous thromboembolism (VTE), defined as thrombotic events without identifiable risk factors 1remains a diagnostic challenge. Multisite idiopathic thrombosis is exceedingly rare and may mimic systemic or malignant conditions. Case Presentation: We report a 32-year-old female with no known thrombotic predisposition who presented with deep vein thrombosis (DVT), pulmonary embolism (PE), and renal vein thrombosis. The patient had a background of controlled type 2 diabetes, hypothyroidism, and rheumatoid arthritis but no family history of thrombosis or recent triggering events. Comprehensive thrombophilia and autoimmune evaluations were negative. She was managed with unfractionated heparin followed by long-term oral anticoagulation (Apixaban), with favourable clinical response. This case highlights the importa Conclusion: nce of recognizing idiopathic multisite thromboembolism as a distinct clinical entity and outlines a structured approach to diagnosis and management. Longterm anticoagulation with DOACs may offer effective and safe outcomes in such cases.
