Orthodontic Treatment of Impacted Maxillary Canines: A Five-Year Retention Case Report

Introduction: Maxillary canine impaction presents a significant challenge in orthodontics, with the optimal treatment approach still under debate. This study aims to detail the management process of maxillary canine impaction in a young female patient. Case Report: A 12-year-old female presented to our dental unit seeking orthodontic care. Clinical examination revealed a Class I molar relationship and the presence of both upper deciduous canines. Panoramic X-ray and PA X-ray using the SLOB technique showed bilateral buccal impactions of the permanent canines, with the crowns of the impacted canines overlapping the roots of the lateral incisors. Surgical exposure of the canines was performed under general anesthesia, during which attachment and traction wires were placed. Results: The treatment concluded with the patient achieving a Class I canine relationship, normal overjet and overbite, and well-aligned dental arches. Radiographic evaluation indicated significant root parallelism and maintained periodontal health in the canine regions. Conclusion: Managing impacted maxillary canines is always a challenge for orthodontists. Successful management is crucial for both aesthetic and functional reasons. Ongoing assessment and early intervention can help prevent adverse situations. Proper treatment planning, effective space management, and the application of light, continuous traction forces are the key factors determining the success of each impacted case.
