Psychological and Health-Related Issues Faced by the Transgender Community in Pakistan

Background: This study examines the perceived psychological and health-related challenges faced by the transgender community in Punjab, Pakistan. Methods: A cross-sectional design was employed, collecting data from 331 transgender individuals across the districts of Lahore, Rawalpindi, and Faisalabad through face-to-face interviews using a structured questionnaire. Proportional stratified and snowball sampling techniques were utilized to access this hard-to-reach population. Results: Quantitative data analysis, using descriptive statistics, revealed a strong consensus on severe health disparities (Mean=4.00). Critical mental health issues were highly prevalent, with depression (Mean=3.89) and anxiety (Mean=3.38) ranking as the most significant concerns. Participants also reported substantial barriers within the healthcare system, including a lack of primary care (Mean=2.74), discrimination by doctors (Mean=2.51), and unsatisfactory treatment in hospitals (Mean=2.37). Furthermore, issues of internalized stigma, such as negative self-image (Mean=2.76), and alarming rates of attempted suicide (Mean=2.59) were identified. Conclusion: The findings underscore a dual crisis of mental health distress and systemic healthcare exclusion. The study concludes that the transgender community in Pakistan navigates a hostile healthcare environment that fails to address their basic needs, exacerbating psychological distress. There is an urgent need for policy reforms focused on healthcare provider sensitization, the establishment of affirming health services, and targeted mental health interventions to mitigate these disparities and promote health equity.
