A42 Comparison of the efficacy of endovascular treatment and surgical clamping in patients with intracranial unruptured aneurysms by platelet function testing
<h3>Introduction</h3> To compare the clinical efficacy of craniotomy and endovascular embolization in the treatment of patients with high-risk ruptured intracranial aneurysms. <h3>Aim of Study</h3> To compare the clinical efficacy of craniotomy and endovascular embolization in the treatment of patients with high-risk ruptured intracranial aneurysms. <h3>Method</h3> Eighty-five patients with high-risk ruptured intracranial aneurysms treated from February 2022 to June 2023 were divided into control (43 cases) and observation (42 cases) groups using a randomized numerical table. The control group underwent surgical clamping, while the observation group received endovascular treatment. Platelet function indexes (PLT, MPV, PDW, PCT) and brain damage markers (S100B, NSE) were compared preoperatively, 6 h, and 24 h postoperatively. <h3>Results</h3> Preoperatively, PLT, MPV, PDW, and PCT showed no significant difference between groups. At 6 h and 24 h postoperatively, the observation group had lower PLT (3×1011/L vs 2×1011/L), MPV (10.0 fL vs 8.0 fL), PDW (18% vs 15%), and PCT (0.3% vs 0.2%) than the control group (P<0.05). Preoperative S100B and NSE levels were similar (P>0.05). At 6 h and 24 h postoperatively, the observation group had lower S100B and NSE levels than the control group (P<0.05). <h3>Conclusion</h3> Endovascular treatment and surgical clamping are both effective clinical treatments for high-risk ruptured intracranial aneurysms, with endovascular treatment causing less brain damage to patients. <h3>Conflict of Interest</h3> No
