WORDCLOUD, A NOVEL INFORMATION-GATHERING TOOL TO DETECT BEHAVIOURAL SYMPTOMS IN PATIENTS WITH FRONTAL LOBE GLIOMAS
Abstract AIMS Frontal lobe gliomas often cause personality/behavioural changes. These range from subtle to very challenging for both patients and relatives. Subtle symptoms may be unrecognised, whereas challenging behaviour change may be underreported by relatives who fear distressing the patient, who may lack insight. This can result in missed opportunity for optimisation of symptom management and support. The Wordcloud tool is a visual representation of words/phrases describing behavioural changes, displayed as an image in the shape of a cloud. The aim is to enhance symptom detection during consultations for patients with frontal lobe gliomas. METHODS The words/phrases in the Wordcloud tool were assessed for readability and found to be suitable for most 15- 16-year-olds. A focus group of patients reviewed the tool to ensure its relevance and ease of use. Between July and December 2024, 21 patients with frontal lobe gliomas were identified at the Royal Marsden. During their consultations, patients/relatives circled relevant symptoms. When applicable, patients received a supportive leaflet and Occupational Therapy input. Questionnaires were completed by patients/carers, and medical staff to assess the tool’s effectiveness. RESULTS All 21 patients found the tool easy to complete and understood its purpose. Fourteen patients found it helpful, two were unsure, and one did not find it useful (four did not answer). Three patients had difficulty under- standing “emotional lability,” and one suggested adding “mental fatigue.” Medical staff found the tool useful for obtaining additional information during history-taking, with most finding it easy to interpret. CONCLUSION The Wordcloud tool is effective for capturing behavioural symptoms in patients with frontal lobe gliomas that may otherwise be overlooked. It facilitates improved symptom management and rehabilitation, particularly through Occupational Therapy, ultimately leading to better holistic care for both patients and their families.
