The Specialist Nurse role in the management of a complex multivisceral transplant case
Background: Addenbrooke’s Hospital Cambridge multi-visceral transplant service receives referrals nationally and internationally. Following transplantation, patients are discharged home but care is managed lifelong by the transplanting team. Inevitably this can be challenging and requires extra support and considerations. The specialist nurses are pivotal in care coordination from a distance.
