Personalized Immunosuppressive Strategies in Intestinal Transplantation: A Predictive Model for Graft Survival and Patient Outcomes

NMP maintains epithelial architecture during 6H of storage with reduced damage, compared to CS, following in vivo reperfusion.NMP also filters lactate throughout 6H storage. We hypothesized that NMP preserves epithelial cell integrity and intestinal barrier over prolonged periods of at least 12H.Methods: Small intestines from 8 pigs were NMP-preserved for 12H.Jejunal biopsies were obtained throughout NMP.Histological injury was assessed using the Chiu-Park grade.During perfusion, hourly blood and waste lactate levels were measured using enzymatic amperometry. Results: NMP preserves jejunum for 12H.Chiu-Park scores increased slightly at 6H, though to a score still viable for transplantation (1), and remained overall stable through 12H (Figure 1A).NMP efficiently clears lactate.Following an initial stabilization period, lactate in the blood and waste gradually decreases over time with consistently higher lactate levels in waste dialysate (Figure 1B).Conclusions: NMP preserves jejunal epithelial integrity for up to 12H while efficiently clearing lactate. This promotes metabolic homeostasis, reduces ischemia-related stress, and extends viable storage time, increasing organ availability for transplantation.Overall, these results highlight the potential of this innovative preservation method to enhance transplantation success and ultimately improve patients' quality of life.
