The Role of Occlusal Splints in Management of Temporomandibular Disorders

Temporomandibular joint (TMJ) disorders constitute a complex set of conditions characterized by pain, joint sounds, restricted jaw movement, and functional impairment, often linked to parafunctional activities and occlusal discrepancies. Occlusal splints, also known as occlusal appliances or night guards, have gained widespread acceptance as a conservative and non-invasive treatment modality in managing TMJ dysfunctions. These splints function by stabilizing the occlusion, reducing abnormal muscle activity, and minimizing joint loading, thereby alleviating pain and improving mandibular function. Various types of splints-such as stabilization splints, anterior positioning splints, and pivot appliances are tailored to individual patient needs and specific TMJ pathologies. Clinical evidence supports their efficacy in reducing myofascial pain, joint sounds, and hyperactivity of masticatory muscles, often serving as an adjunct to behavioral therapy, physical therapy, and occlusal adjustment procedures. Despite their benefits, the success of occlusal splints relies heavily on accurate diagnosis, proper fabrication, and patient compliance. Additionally, ongoing research continues to elucidate the mechanisms underlying their therapeutic effects and optimize their design. Overall, occlusal splints play a pivotal role in a multidisciplinary approach to TMJ disorders, offering symptomatic relief and functional restoration when integrated within comprehensive management protocols.
