ESTRATÉGIAS E INTERVENÇÕES DE PREVENÇÃO DE COMPLICAÇÕES DECORRENTES DA RUPTURA PREMATURA DE MEMBRANAS EM GESTAÇÕES DE ALTO RISCO

Introduction: Prelabor rupture of membranes (PROM), defined as the spontaneous rupture of fetal membranes before the onset of labor and generally before 37 weeks of gestation, is a significant obstetric challenge. This complex and multifactorial condition, especially prevalent in developing countries, is a leading cause of prematurity and neonatal and maternal morbidity and mortality. The study aims to explore strategies and interventions to prevent PROM complications in high-risk pregnancies, highlighting their benefits for both the pregnant woman and the fetus. Methodology: This was an integrative literature review. Searches were conducted in the PubMed and LILACS (via VHL) databases. The sample consisted of 25 national and international studies published between 2019 and 2025. Results and Discussion: PROM is a prevalent condition (2-3%, and up to 6.58% in developing countries) and a major contributor to preterm birth. Risk factors include infections (such as UTIs and cervicovaginitis), malnutrition, vaginal bleeding, obstetric history (abortion, preterm birth), poorly controlled gestational diabetes, and exposure to air pollution. Interventions such as antibiotic therapy, tocolysis, and vitamin C supplementation were analyzed. Conclusion: PROM management requires a multifaceted approach. Early identification and management of risk factors, especially infections and metabolic imbalances, are crucial. Antibiotic therapy is an effective and recommended intervention. Although some interventions show potential, the complexity of PROM and the absence of universal predictive models still present challenges. Further research is needed to optimize treatment protocols and fill existing gaps, aiming to improve maternal and neonatal outcomes.
