Association of the ischemic heart disease with blood pressure, age and work experience in men with shift work in the Far North

Expeditionary rotation organization of labor in the industry of the northern regions requires higher adaptive capabilities in individuals related to the extended stays in the North and age, with the risk of integration into cardiovascular diseases, with the generalization of coronary heart disease (CAD). Objective. To determine the features of the association of the prevalence of coronary heart disease, the blood pressure, work experience, and age in men aged 25—54 years, working on a rotational expeditionary basis in the Tyumen North. Materials and methods. The study included men aged 25—54 working on an expeditionary rotational basis in the Tyumen North. A representative sample, based on independent numbers, included 750 subjects with a response rate of 82.4%, equally distributed across age groups (25—34, 35—44, and 45—54 years) and work experience. The study used unified epidemiological methods to assess the blood pressure and the presence of coronary artery disease. Statistical analysis was performed using the Statistica 12 program, and p<0.05 was considered critical. Results. In the study population of men working on a rotational expeditionary basis, a high prevalence of coronary artery disease, according to extended criteria, was revealed — 9.4% («confirmed» coronary artery disease in 3.4% and «possible» coronary artery disease in 6.0%). A correlation was found between the prevalence of coronary artery disease and the level of blood pressure, age, and work experience in the North. In the presence of hypertension, the chance of developing coronary artery disease according to extended criteria increased more than 3 times (OR 3.191, 95% CI 1.808—5.630), in case of work experience in the North of 10 years or more, 2.5 times (OR 2.585, 95% CI 1.15—5.84), and when reaching the age of 45—54 years, more than 6 times (OR 6.350, 95% CI 2.749—14.665). Conclusion. An integrated approach to studying the relationship between the prevalence of coronary heart disease and arterial hypertension, age, and work experience in the North is a reasonable and promising direction for coordinating preventive programs in the northern regions.
