Spectrum of Acute Kidney Injury and its Outcome in Chronic Liver Disease Patients in the Emergency Department

Background: Patients with chronic liver disease (CLD) frequently have acute kidney injury (AKI), a significant consequence that raises morbidity and mortality. The complex interplay of liver dysfunction, circulatory disturbances, and renal impairment poses a significant clinical challenge. This study aims to evaluate the incidence, risk factors, and outcomes of AKI in CLD patients to enhance early diagnosis and management strategies.Patients and methods: A prospective observational study was conducted on patients with CLD admitted to the hospital over a specified period.Patients diagnosed with AKI based on the KDIGO criteria were included.Information was gathered about clinical outcomes, renal function metrics, liver disease etiology, and demographic traits.Statistical analysis was performed to identify significant predictors of AKI and its impact on patient prognosis. Results: The incidence of AKI among CLD patients was found to be significant, with a higher prevalence in those with advanced cirrhosis and sepsis.Key risk factors included infections, hypovolemia, and use of nephrotoxic drugs. The mortality rate was notably higher in patients who developed AKI, particularly those requiring renal replacement therapy. Conclusions: Acute kidney injury in CLD is associated with poor prognosis, emphasizing the need for early recognition and prompt intervention.Identifying modifiable risk factors can help improve patient outcomes.Future research should focus on refining therapeutic approaches and preventive strategies to reduce the burden of AKI in CLD patients.
