A Step towards Effective Blood Management: Prevention, Early Diagnosis, and Minimization Strategies for Anemia

Introduction: Blood transfusion is a life-saving intervention but carries risks of adverse reactions and transfusion-transmitted infections.Effective blood management strategies, including prevention, early diagnosis, and minimization of unnecessary transfusions, are essential for optimizing patient care.Aim and objective: This study aims to assess the clinical appropriateness of blood transfusions and optimize their use for better patient care. The objective is to evaluate packed red blood cell (PRBC) utilization, reduce unnecessary transfusions, and promote guideline to enhance efficiency and safety.Materials and methods: This study was conducted at the Blood Center, Jawaharlal Nehru Medical College, Ajmer, Rajasthan, to evaluate the appropriateness of PRBC transfusions.A retrospective cross-sectional study of blood supply patterns over the course of 6 months from January 2023 to June 2023 was performed using requisition forms and blood issue registers. Results: A total of 7,256 PRBC units were transfused to 4,618 patients. The most common blood group supplied was B-positive (35.04%), and the highest number of transfusions was observed in patients aged above 50 years. The leading indication for transfusion was anemia (57.80%), followed by thalassemia (13.27%).Notably, 41.67% of transfusions were administered to patients with hemoglobin (Hb) levels between 7.1 and 10 gm/dL.Single-unit transfusions constituted 63.70% of cases. Conclusion:The study emphasizes judicious blood use, guideline adherence, and alternative strategies like anemia correction and surgical conservation.Regular audits and clinicians training or awareness programs can improve transfusion practices, enhancing patient safety and resource efficiency.
