Patient-centered medical home and the quality of primary care: survey study with patients and administrators of community healthcare centers in Shanghai, China
The study validated PCMH as a robust framework for assessing primary care performance. Furthermore, it identified specific areas where CHCs can enhance the patient experience, particularly in the domains of Coordination (Information Systems), Care Coordination, and Care Transitions. These findings underscored the need for targeted interventions to strengthen these aspects of primary care delivery, ultimately improving patient outcomes and satisfaction.
