The publisher regrets that an error was made with the upload of the supplementary material. The CONSORT 2010 checklist page numbering is incorrect and relates to a previous typesetting of the article. A revised document correcting the page that each checklist item has been reported on is to replace this. The publisher would like to apologise for any inconvenience caused.
