"My Care Plan" for patients with upper gastrointestinal cancer – digital knowledge support for patients

Abstract Introduction The "My Care Plan" initiative was developed to enhance support and information access for patients with cancer in Sweden, addressing disparities in care across the country’s 21 healthcare regions. The digital tool aims to increase patient participation and promote person-centred care through structured, diagnosis-specific information and resources. The project responds to challenges with variations in access to psychosocial support and rehabilitation for patients with cancer. Method The methodology for all My Care Plan involves collaboration among national working groups, including clinical experts and patient representatives, to create a library of tailored, evidence-based content. This content includes texts, videos, and self-assessment tools to meet individual needs across diagnosis and the care continuum. My Care Plan also offers asynchronous messaging with professionals, providing flexibility and continuity in communication. A nationwide survey was distributed to patients and healthcare providers as an initial evaluation. My Care Plan for pancreatic cancer launched in May 2023, for oesophageal and gastric cancers in November 2024, and by autumn 2025 a launch for hepatobiliary cancers is planned. Result For pancreatic cancer, oesophageal and gastric cancers, over 1400 care plans have been initiated by nurses nationwide, although adoption rates vary by region. Initial feedback indicates that My Care Plan improves patient satisfaction, reduces the administrative burden on healthcare providers, and supports patients’ confidence in self-care. Discussion While My Care Plan demonstrates potential to improve care continuity and patient autonomy, challenges remain, such as balancing standardization with personalization. Further, its unequal implementation nationwide warrants further actions.
