Background: Whether asymptomatic or symptomatic, flexible flatfoot (FFF) is defined by a normal arch while not bearing weight and a flattening of the arch when stanced. Even while flatfoot seldom results in disability, some kids have diminished lower limb function and experience lower limb pain.Aim: to assess the outcomes of osteotomy for lateral calcaneal lengthening with tibialis posterior advancement in treatment of symptomatic flexible flatfoot deformity.Patients and methods: This was a clinical study that involved patients visit pediatric orthopaedic unit in El Nasr specialized hospital in Port Said. The study was conducted on 20 patients. Pre- and post-operative pain and ankle-hindfoot scores from the American Orthopaedic Foot & Ankle Society (AOFAS) were compared. Results: Preoperative mean AOFAS score was 55.8 ± 6.3; postoperative mean score was 91.1 ± 3.2 (p < 0.001). 60% of patients reported total pain alleviation at the final follow-up. while 40 % experienced only occasional discomfortConclusion: Clinically, the majority of patients reported complete relief or significant reduction in pain.
