Practical aspects of antiplatelet therapy individualization among patients undergoing PCI for ACS and CCS. The joint expert opinion of the Working Group on Cardiovascular Pharmacotherapy and the Association of Cardiovascular Interventions of the PCS

Dual antiplatelet therapy remains the cornerstone of medical therapy among patients undergoing percutaneous coronary interventions for acute coronary syndrome or chronic coronary syndromes. Dual antiplatelet therapy is instrumental in reducing the risk of thrombotic events. However, antiplatelet agents increase the risk of bleeding. The current approach has therefore focused on assessment of individual risk factors and turned to tailor-made therapies rather than a "one-fitsall" approach. The goal of the present document is to overview the currently available antiplatelet strategies post percutaneous coronary interventions, with particular focus on patients' bleeding and ischemic risk factors as well as comorbidities and varied clinical scenarios, including patients at advanced age, patients with chronic kidney disease, diabetes, cancer, extreme body weight, and patients undergoing complex coronary procedures.
