Documentation in Physiotherapy Practice: A Narrative Review
Physiotherapy is a dynamic process with a strong theoretical foundation and a broad range of clinical applications in the development and restoration of optimal function. The scope and practice of physiotherapy have outgrown the complexity. The advantages of documentation are that it provides the documentation for the services rendered, assists in maintaining care continuity, multidisciplinary collaboration is based on a shared health record. An audit allows for the evaluation of documentation standards, which aids in the detection and improvement of medical record documentation. The implementation and utilization of electronically produced documentation could aid physiotherapists in better organizing their notes. Overall, the grade of physiotherapy documentation in in-patient medical records was determined to be satisfactory, with a few particular problems requiring improvement. To ensure that professional and regulatory standards are met, and to improve communication between health providers, consistent high-standard medical record documentation is required.
