Exploring the Role of Pharmaceutical Care in Identifying and Controlling Modifiable Risk Factors among Stroke Patients

Introduction: Stroke is a multifaceted disease driven by modifiable and unchangeable risk factors. This study investigates the potential for reducing the severity of stroke's impact by actively managing modifiable risk factors and ensuring consistent medication adherence. Method: This study employed a prospective interventional design, enrolling 100 patients aged 18 years or older, both male and female, who were either inpatients or outpatients with a confirmed stroke diagnosis. The patients received the intervention, and both the identification of modifiable stroke risk factors and their medication adherence were assessed at the initial evaluation and during monthly follow-up visits. Result: Out of 100 enrolled patients, the modifiable risk factors were hypertension and diabetes mellitus. There was a considerable improvement after the intervention in SBP (F=204.075, p =0.0001), DBP (F=33.5030, p=0.0001), FBS (F=2013.31, p =0.0001), HbA1c (p =0.0001) and medication adherence (F=903.9460, p=0.0001). Conclusion: Post-analysis confirmed the successful implementation of interventions aimed at controlling modifiable risk factors and enhancing medication adherence. These improvements underscore the significant benefits of pharmaceutical care provided by clinical pharmacists. The study's conclusion strongly recommends the integration of pharmaceutical care into stroke patient management, as it optimizes patient care, controls risk factors, and ultimately, extends life expectancy.
