Relation of qSOFA Score to Predict Outcome in Patients with Peritonitis

Introduction: A scoring system with various parameters is the foundation on which peritonitis is managed. Numerous grading methods have been created to assess a disease's prognosis. This study's objective is to demonstrate the connection between quick Sequential Organ Failure Assessment (qSOFA) score and outcome in patients with peritonitis. In the emergency room, we tested the feasibility of a quick, easy-to-calculate score (qSOFA), which is based on three binary clinical variables. Methods: This is a prospective observational study of done between 1st November 2021 to 30th of Mrach 2023 with total duration of 17months done in Dhulikhel Hospital , Kathmandu University Hospital, after taking ethical consent from the institution Review committee. The discussion of qSOFA score and outcome in patient with peritonitis were studied. All the patient were above 18years, and admitted to Department of Surgery were included. Results: This study had 71 patients in total, aged between 19 and 89 years, with a mean age of 47.6 years and a male-to Female ratio of 2.7:1. The total number of patients admitted to the ICU was 26. Higher qSOFA was associated with the requirement for ICU admission and a longer ICU stay, with p 0.005 and 0.024, respectively. However, higher qSOFA was not statistically significant with length of hospital stay, need for mechanical ventilation, or mortality, with p values of 0.39, 0.31, and 0.20, respectively. Conclusion: This study showed that qSOFA is a good scoring system that helps in determining the intensive care unit (ICU) admission and length of ICU stay. But this study does not show statistical significance in relation to length of hospital stay, need for mechanical ventilator support, or mortality.
