A case of ossifying fibroma of the mandibular condyle treated by a condylectomy and reconstruction using a vertical ramus sliding osteotomy
Ossifying fibromas are benign fibro-osseous lesions, commonly involving the posterior mandible in middle-aged individuals, and have a female predilection.Ossifying fibromas in the mandibular condyle are rare. Here, we report a case of ossifying fibromas in the mandibular condyle treated with a condylectomy and reconstruction using the vertical ramus sliding osteotomy technique.A 50-year-old woman was referred to our department by a family doctor because of discomfort in the right temporomandibular joint and pain on mandibular movement.CT and MR images revealed a solitary lobulated mass with well-defined borders and no soft tissue invasion in the right condylar head.99m Tc-MDP accumulation was observed in the same area.A surgical procedure using an Al-Kayat incision and a high perimandibular approach was performed to excise the condyle. After the condylectomy, a 10 mm vertical defect was observed and reconstructed using a vertical sliding osteotomy of the mandibular ramus.Rigid internal fixation was achieved using two titanium locking miniplates. The histopathological diagnosis was ossifying fibroma.Intermaxillary elastic traction was continued for one month. One month postoperatively, good pain reduction, good mouth opening, and no deviations or malocclusions were observed. No recurrence or jaw dysfunction was noted three years postoperatively.
