Low Rates of Early Aseptic Loosening of the Cemented Attune S+ Total Knee Arthroplasty System

Background Aseptic loosening is a common cause of late revision surgery performed following total knee arthroplasty (TKA). Recent advances in implant designs have aimed at reducing this risk, with the cemented Attune S+ total knee system focused on improving stability through enhanced cement adherence. Despite excellent preliminary in vitro data, there is a lack of clinical studies assessing the performance of this implant, with prior studies reporting high rates of aseptic loosening. The primary objective of this study was to assess the clinical outcomes, primarily rates of aseptic loosening and revision, of Attune S+ implants. Methodology This retrospective study included 47 patients who underwent TKA using the cemented Attune S+ system between November 2017 and April 2023. All surgeries were performed by a single fellowship-trained arthroplasty surgeon at a tertiary care academic medical center. The primary outcome was aseptic loosening, which was evaluated on postoperative knee radiographs for stress shielding and radiolucent lines. Results There were no patients with radiographic evidence of aseptic loosening, and no known revision surgeries were performed. Of note, one patient had a subsequent operation 817 days after the original TKA to remove scar tissue in the knee joint due to arthrofibrosis, but no signs of prosthetic failure were found intraoperatively. Conclusions In this study, there were no cases of aseptic loosening or revision surgery observed with the cemented Attune S+ TKA system.
