Leveraging interdisciplinary collaboration and team‐based care to develop a standardized model for the assessment of medication adherence and barriers
ABSTRACT A standardized, interdisciplinary care team approach for monitoring medication adherence and interventions across ambulatory specialty clinics within a pediatric hospital is necessary to achieve optimal treatment outcomes. Although individual health care providers from distinct disciplines routinely assess and intervene to improve adherence in patients with complex medication regimens, our institution has not historically had an integrated, collaborative model for standardizing these efforts across disciplines. The purpose of this institutional improvement project was to apply an interprofessional, team‐based approach to standardize the measurement of medication adherence barriers and the documentation of medication adherence improvement interventions.
