A CLINICAL CASE OF PROGRESSIVE VASCULAR LEUKOENCEPHALOPATHY WITH FREQUENT EPISODES OF VERTEBROBASILAR INSUFFICIENCY AND EXTRAPYRAMIDAL MANIFESTATIONS
The article presents a clinical case of progressive vascular leukoencephalopathy (Binswanger's disease) in a 60-year-old female patient with a long-term history of hypertension, episodes of vertebrobasilar insufficiency and the presence of concomitant diseases (type 2 diabetes mellitus, nodular goiter, frontal lobe meningioma). The clinical case includes hyperkinesis of the "hemiballismus – hemichorea" type, cognitive impairment of the subcortical type, frontal dysbasia and conditions associated with vascular lesions. Particular attention is paid to the diagnostic approach, including neuroimaging and multimodal research methods, as well as the successful use of complex drug therapy (neuroleptics, anticonvulsants and tranquilizers). The conclusions emphasize the need for an individual approach, compliance with prevention methods for this category of patients, which allows for effective relief of hyperkinetic syndromes and improvement of quality of life.
