Primary Reconstructive Management of Severe Ocular Trauma: Case Report
Severe periocular trauma without globe involvement represents a reconstructive challenge that requires timely surgical intervention to restore eyelid function, facial symmetry, and prevent aesthetic and functional sequelae. This article presents the case of a 69-year-old male patient who sustained an extensive injury to the left periorbital region as a result of a work accident involving a table saw. Despite the high-energy trauma, no intraocular damage or direct globe involvement was detected, with ocular integrity confirmed through ophthalmologic evaluation. Surgical management was performed by the plastic surgery team, consisting of primary reconstruction including soft tissue repair, layered closure, and eyelid margin reconstruction. Early intervention preserved eyelid functionality, prevented secondary deformities, and achieved a satisfactory aesthetic outcome. This case highlights the importance of a multidisciplinary approach and prompt intervention in facial traumatic injuries, even in the absence of direct ocular damage, to optimize anatomical and functional recovery of the periorbital area.
