Investigating the Correlation between Electrocardiographic Parameters and the Occurrence of Clinical Symptoms in Patients with Idiopathic Premature Ventricular Contractions

Abstract Background: This study aimed to investigate the correlation between electrocardiographic (ECG) parameters of premature ventricular contraction (PVC) morphology and clinical outcomes, including symptom burden and health-related quality of life (HRQoL). Materials and Methods: This observational study at Rajaie Cardiovascular Center examined the relationship between PVC ECG parameters and HRQoL in patients undergoing ablation for idiopathic PVCs. Data on demographics, echocardiography, and ECG parameters were collected. The study received ethical approval, and statistical analysis was performed using IBM SPSS Statistics version 27. Results: The study included 50 patients (46% male, 54% female; mean age 50.4 years). The prematurity index showed a moderate positive correlation with symptom burden ( r = 0.386, P = 0.01), whereas the post-PVC pause exhibited a strong negative correlation with symptom burden ( r = −0.627, P < 0.01). PVC duration positively correlated with HRQoL ( r = 0.335, P = 0.03). Other parameters such as coupling interval, retrograde P -waves, interpolated PVCs, and PVC morphology did not show significant correlations with outcomes. Conclusions: The findings suggest that specific ECG parameters – particularly the prematurity index, post-PVC pause, and PVC duration – are associated with symptom burden and HRQoL in patients with idiopathic PVCs. These results highlight the importance of considering these parameters in the evaluation and management of PVC patients to enhance overall well-being. Further research is recommended to validate these findings and explore underlying mechanisms.
