A Clinical, Cephalometric, and Electromyographic Profile of AdvanSync-2 Class II Molar to Molar Appliance for the Correction of Skeletal Class II Malocclusion with Orthognathic Maxilla and Retropositioned/Retrognathic Mandible

Objectives: Class II malocclusion can be effectively managed with fixed functional appliances. For noncompliant, postpubertal patients who do not later require orthognathic surgery, this is the only effective bite-jumping treatment. According to the reports, Class II skeletal issues can be resolved with fixed functional appliances by promoting mandibular growth and eliciting dentoalveolar effects. Hence, in this study, an attempt has been made to compare the efficacy of AdvanSync-2 appliance with Class II elastics in the correction of Class II malocclusion. Materials and Methods: A cross-sectional, prospective, and clinical study was conducted in the Department of Orthodontics and Dentofacial Orthopedics for 18 months. Approval from the ethical committee of the university was obtained for the study. The sample size of the study constituted of 10 subjects having skeletal Class II malocclusion, with an age range of 14–18 years. All patients had signed consent forms permitting the use of their records for research. The subjects were treated with AdvanSync-2 Class II Molar-To-Molar appliance using the traditional mode of attachment where the lower dental arch was the anchorage source. Results: The study showed that the AdvanSync-2 appliance works well in clinical practice. Patients showed clear improvement in their facial profiles on examination. It was observed that there is a significant difference in the mean values of electromyographic values (µv) of left temporalis muscle and left masseter muscle during maximum voluntary clenching before treatment with AdvanSync-2 appliance (0 th month’ evaluation) and after treatment with AdvanSync-2 appliance (6 months’ evaluation) ( P = 0.0001). It was observed that there is a significant difference in the mean values of assessment of cephalometric changes before treatment with AdvanSync 2 appliance (0 th month evaluation) and after treatment with AdvanSync 2 appliance (6 months evaluation) ( P = 0.0001). Conclusion: Advansync-2 appliance is highly efficacious in causing protraction of mandible and improvement in the patient’s overall facial profile.
