Analysis of Risk Factors for Contralateral Hip Refracture after Hip Fracture in the Elderly

Abstract Objectives: To investigate the incidence of contralateral hip refracture and refracture in the elderly, and to provide clinical basis for early intervention treatment and prevention of refracture in geriatric medicine. Materials and Methods: After analyzing the clinical data of hip fracture patients admitted to our hospital from January 2019 to January 2022 and obtaining 160 elderly patients with hip fracture, they were divided into refracture group and non-refracture group. Relevant conditions during admission and discharge follow-up were collected to record the related factors that might affect contralateral hip refracture after hip fracture. R 4.1.3 statistical software was used for data analysis, and the different factors in univariate analysis were included in the multivariate Logistic regression model. Results: Among the 160 patients followed up, 15 patients had contralateral refracture within 3 years and 145 patients without refracture, of which the incidence was 4.37% within 1 year and 9.37% within 3 years. Compared again fracture group and non-fracture group basic characteristics and their basic disease, the results show between the two groups in age, osteoporosis, cardiovascular disease, knee osteoarthritis (K-L grade III-grade), dementia, statistical significance ( P < 0.05), compared with non-fracture group, fracture group patients older, with osteoporosis, cardiovascular disease, knee osteoarthritis (K-L grade III-grade), dementia proportion is higher. Including the multivariate logistic regression analysis (B (SE) = 2.16 (1.08) OR (95%CI) = 8.71 (1.06, 71.65) P = 0.044), cardiovascular disease (B (SE) = 1.73 (0.81) OR (95%CI) = 5.67 (1.15, 27.94) P = 0.033), Dementia (B (SE) = 1.71 (0.64) OR (95%CI) = 5.54 (1.57, 19.52) P = 0.008) was a risk factor for refracture of the contralateral hip within 3 years. Conclusion: The risk factors of contralateral hip refracture in the elderly include osteoporosis, dementia, cardiovascular-related diseases, etc. The corresponding intervention methods should be rationally applied to regulate the high-risk factors, reduce the incidence of contralateral hip refracture, and improve the quality of life of the elderly.
