Managing Right-Sided Recurrent Herniation through the Left-Sided Interlaminar Approach Via the Over-the-top Decompression Technique using Unilateral Biportal Endoscopy: Key Techniques and Insights

Abstract Unilateral biportal endoscopy is a widely used minimally invasive spine surgery that is used to treat various spine pathologies such as disk herniation, spinal canal stenosis, spinal tumor, and interbody fusion. This surgical method offers numerous benefits for patients when compared to conventional open surgery and other minimally invasive techniques. These include very high magnification, minimal blood loss, less tissue damage, faster recovery, and better cosmetic healing. It is widely used in the treatment of disk herniation apart from other spinal pathologies. It has the additional advantage of removing disk fragments in contralateral recurrent disk herniation cases via the over-the-top decompression technique at the same level without causing any additional damage to the normal posterior osseo-ligamentous structure. This article describes a technique for approaching contralateral recurrent disk herniation from the left side without damaging normal posterior structures, specifically for right-handed surgeons.
