Relation of Hormonal Status with Clinical Features among Patients with Polycystic Ovarian Syndrome Attending in a Tertiary Care Hospital of Bangladesh.

Polycystic ovary syndrome (PCOS), a common and heterogeneous disorder, is characterized by oligo or anovulation, menstrual disturbances, and androgen excess. PCOS usually begins at puberty and manifested by hirsutism, obesity, insulin resistance, acanthosis nigricans, acrochordons and menstrual irregularities. Aim of the study was to assess the relation between clinical features of PCOS and different hormonal level pattern. This cross-sectional descriptive type of observational study was conducted in the Department of Obstetrics & Gynaecology and Endocrinology of Mymensingh Medical College Hospital (MMCH), Bangladesh from the period of January 2018 to December 2018. Total 108 patients were included for the study. Sample was selected by purposive non randomized sampling. Data were processed by using the Statistical Package for Social Sciences version 23.0. Ethical clearance was taken from the Institutional Review Board (IRB) of MMC. Majority (44.4%) patients belonged to age group 21-30 years with mean age was 23.8±6.2 years. Majority (57.4%) patients were married. Majority (91.7%) patients had hirsutism followed by 40(37.0%) acne, 40(37.0%) acanthosis nigricans, 16(14.8%) weight gain, sub- fertility. In this study 03(2.8%) patients were found underweight, 40(37.0%) overweight and 11(10.2%) were obese. The mean Hirsutism, Modified Ferriman Galway score was found 10.5±2.9 with range from 4 to 20. More than three fourth (75.9%) had oligo menorrhea, 20(18.5%) amenorrhea and 6(5.6%) patients had menorrhagia. Eighty (74.1%) patients had normal thyroid and 28(25.9%) had hypothyroid. Sixty (55.6%) patients were found normal HDL and 48 (44.4%) were found high HDL level. Mean serum testosterone was found 1.28±0.22 ng/ml. On USG of Pelvic Organs 27(27.0%) patients had unilateral and 73(73.0%) had bilateral PCOS (ovary) and 08(7.4%) patients had no PCOS on USG. Serum testosterone level 68(93.15%) was higher in bilateral group. Based on the findings of the study concluded that early detection of specific clinical features related to PCOS and effective screening of hormones (FSH, LH, TSH, Prolactin and Testosterone) and sonographic findings can detect the PCOS patients at early stage and thus can reduce the PCOS related symptoms and infertility.
