SURGICAL TREATMENT OF LARYNGEAL CANCER FROM THE STAND-POINT OF PREVENTION OF ENDOCRINOLOGICAL DISORDERS

Operable laryngeal cancer at late stages is treated in most cases by total laryngectomy, including resection or complete removal of the thyroid gland. This approach is justified by the fact that the thyroid gland, due to its close topographic anatomical adjacency to the larynx, can be affected by the tumor both by contact and by metastases. Such tactics can lead to postoperative hypothyroidism and hypoparathyroidism. The need for thy-roidectomy in total laryngectomy, in the opinion of the authors of the article, is a controversial issue. The aim of the study was to determine the need for combined operations of this kind. Objectives of the study: to assess the prevalence of the tumor process, the incidence of postoperative hypothyroidism, and to develop indications for combined interventions for laryngeal cancer. A retrospective analysis of the case histories of patients with laryngeal cancer T3N0M0 and T4N0M0, who received treatment in the Department of Head and Neck Tumors of the State Budgetary Healthcare Institution of the Novosibirsk Region "City Clinical Hospital No. 1" in 2023-2025, who underwent this operation, was performed. During the study, it was established that only 1 of 57 patients had thyroid invasion and the process was locally advanced. The authors concluded that thyroid involvement in laryngeal cancer is rare. Unnecessary he-mithyroidectomy leads to hypothyroidism and hypoparathyroidism. In this regard, the issue of such operations is debatable and subject to further in-depth study.
