Combination of nonpenetrating deep sclerectomy (NDPS) with vasoreconstructive surgery (VRS) in the treatment of glaucoma neuroopticopathy. Clinical observations. To the anniversary of Professor V.I. Kozlov (1930–1996)
Relevance. Glaucoma remains a widespread pathology, occupying leading positions among the causes of blindness. To achieve stabilization of glaucoma process it is necessary to apply pathogenetically directed technologies. Purpose. To present the results of clinical observations confirming the effectiveness of the combination of non penetrating deep sclerectomy (NPSE) with vasoreconstructive surgery (VRS) in the treatment of glaucoma neurooptycopathy. Methods. This paper presents two clinical observations on the use of NGSE with VRO in the complex treatment of glaucoma neurooptycopathy in patients with far advanced glaucoma with increased ophthalmotonus. As the first stage of treatment for normalization of intraocular pressure (IOP), NGSE was performed, followed by VPO, which increased blood supply to the eye. Ophthalmologic examination included visual acuity, IOP measurement according to Maklakov, as well as perimetric studies of the visual field before and after surgery. Results. As a result of the treatment both patients achieved stable normalization of IOP, stabilization of glaucoma process in the remote postoperative period. Conclusion Complex treatment of glaucoma neurooptycopathy including NGSE first stage for IOP normalization and IOP second stage for eye hemodynamics enhancement is an actual direction in glaucoma treatment based on the influence on the main pathogenetic mechanisms of the disease development. Key words: glaucoma neurooptycopathy, vasoreconstructive surgery, ocular hemodynamics
