A clinical case of one-stage surgical treatment of anterior corneal basement membrane dystrophy (Kogan’s dystrophy) by transepithelial photorefractive keratectomy
Purpose. To present a clinical case of one-stage surgical treatment epithelial basement membrane dystrophy in a patient with mild myopia using transepithelial photorefractive keratectomy (Trans-PRK). Methods. In February 2025, a 42-year-old patient applied to Gazprom Eye Microsurgery Clinic with complaints of intermittent pain, lacrimation in the morning from her left eye with exacerbations over the past two years. During the examination, a diagnosis of dystrophy of the anterior basement membrane of the cornea of both eyes was made, and it was decided to perform a left eye PRK. Results. On the 3rd day after the operation, the following data were obtained: in SCL: Vis OS = 0.7. After removing the SCL: Vis OS = 0.5 sph (–)0,75 cyl (–)1.0 ax 109 = 0.7. According to instrumental data, a gentle opacity in the optical zone is probably associated with hyperplasia. One month after the operation: Vis OS = 1.0. According to instrumental data, this is the normal. Conclusion Given the high visual acuity, the absence of complications and endothelial damage, and a marked decrease in aberrations 1 month after surgery, it is possible to recommend single-stage TransPRK in patients with Kogan’s dystrophy as an effective and safe treatment method. Key words: corneal dystrophy, Trans-PRK, eximer laser vision correction
