Dual Use of Type IV Collagen 7S and Ultrasound Elastography in Assessment of Liver Fibrosis in Type II Diabetes Mellitus Patients with Metabolic Associated Fatty Liver Disease
Background: Type IV collagen 7S (T4C7S) plays a crucial role as a biomarker in identifying liver fibrosis among patients with nonalcoholic fatty liver disease.Purpose: To utilize T4C7S in conjunction with ultrasonic elastography to investigate non-invasive approaches for evaluating liver fibrosis in patients suffering from metabolic fatty liver disease and type II diabetes mellitus (T2DM).Patients and Methods: This cross-sectional study was carried out on 200 patients aged from 19 to 65 years old, both sexes, diagnosed with metabolic fatty liver disease, half of the patients have T2DM associated with metabolic fatty liver disease. Patients were divided into two equal groups: Group 1: metabolic fatty liver disease and T2DM and Group 2: metabolic fatty liver disease without DM.Results: A significant correlation was found between the T4C7S and CA fibrosis index scores and the diagnosis of advanced fibrosis in patients classified as groups I and II. There was a significant correlation between the steatosis score and the diagnostic category of group II. There was a significant correlation between the diagnosis of advanced fibrosis in each patient and the hepatic steatosis index. The cutoff value for T4C7S, CA fibrosis index and steatosis index respectively is 2.25, 4.66, 326.5 with a sensitivity of 86.7%, 83.3%, 70.0%, specificity of 87.1%, 91.2% and 60%. In Group II, T4C7S and CA fibrosis scores showed statistically significant differences. Conclusion: Findings indicated that T4C7S and the CA fibrosis index yielded significant outcomes in identifying liver fibrosis, particularly for individuals with metabolic-associated fatty liver disease who had T2DM.
