Blood Staining of Posterior Capsule Following Gonioscopy-Assisted Transluminal Trabeculotomy (GATT) in a Small Pupil
To present a case of delayed-onset posterior capsular blood staining following uneventful gonioscopy-assisted transluminal trabeculotomy (GATT) combined with cataract extraction in a small pupil. A total of 70-year-old male diagnosed case of primary open-angle glaucoma with immature cataracts. He had a preoperative vision of 5/60, intraocular pressure of 26, and a cup disc ratio of 0.8 on two antiglaucoma medications. He underwent uneventful GATT combined phacoemulsification surgery and was discharged on routine post-op medication with pilocarpine eye drops. On POD 43, patients presented with a diminution of vision. His vision was hand movement, intraocular pressure 12, with blood staining of the posterior capsule and a small pupil. Blood staining of the posterior capsule. An Nd:YAG laser capsulotomy was performed on the same day. Post-laser seven-day follow-up patient regained 6/6 vision. By using an Nd:YAG capsulotomy, the blood can be cleared swiftly and safely, allowing the patient’s faster visual recovery.
