Evolução dos dependentes internados num hospital da região Minho de Portugal, referenciados para a rede nacional de cuidados continuados integrados

The present project focuses on the phenomenon of individuals dependent on self-care who are referred to the National Network for Integrated Continuing Care, in a hospital in the Minho region of Portugal. This is a descriptive, exploratory study based on a quantitative paradigm with a longitudinal profile, as it will include four assessment points: 1) Prior to hospitalization; 2) At the time of hospital admission; (3) After referral to the National Network for Integrated Continuing Care, when the patient has a clinical condition suitable for hospital discharge but entry into the network is delayed; (4) Upon admission to the National Network for Integrated Continuing Care. The sample will consist of all individuals referred for this type of care aged 18 years and older. A five-month data collection period has been defined, with an estimated sample size of approximately 200 cases. Data collection will be conducted through access to documented information in the health information system of the National Network for Integrated Continuing Care and the SClínico system. Data processing and analysis will be carried out using the Statistical Package for the Social Sciences (version 19). The study will adhere to ethical principles inherent to the research procedures. The findings of this study are expected to contribute to the generation of evidence supporting a healthcare model that enhances the quality of life of dependent individuals and their families.
