Diabetes and Diabetic Peripheral Neuropathy after Partial Pancreatectomy for Insulinoma in a Golden Retriever
A 7-year-old neutered male Golden Retriever presented with pelvic limb weakness. The patient had been diagnosed with insulinoma and had undergone partial pancreatectomy, after which clinical signs emerged approximately three weeks later.Neurological examination revealed abnormal limb posture with mild proprioceptive ataxia in the pelvic limbs.Blood analysis showed hyperglycemia and increased fructosamine levels, while urinalysis indicated glucosuria.Accordingly, the patient was diagnosed with diabetes and diabetic neuropathy after partial pancreatectomy for insulinoma.Treatment included insulin detemir (0.1 IU/kg, subcutaneously, q 12 h) for diabetes and toceranib (2.5 mg/kg, orally, q 48 h) for insulinoma, which improved neurological symptoms 21 days after treatment initiation. However, on day 101, the patient exhibited hypoglycemia and higher insulin concentrations without exogenous insulin administration.Computed tomography revealed an irregular pancreas and adjacent lymph node enlargement, raising suspicion for recurrence of insulinoma with adjacent lymph node metastasis. The patient experienced persistent hypoglycemia for 4 months and was eventually euthanized at the request of the client. This report offers clinical details of diabetes and diabetic neuropathy following partial pancreatectomy for insulinoma in a dog.
