RELATO DE CASO: CORPO ESTRANHO ACESSANDO DANGER SPACE.

ABSTRACT INTRODUCTION: Cervical perforations by foreign bodies are uncommon, especially in the retropharyngeal space – danger space. Its surgical approach, as well as the technique used, depends on the topography and degree of lesion of the cervical viscera and the development of signs and symptoms of cervical- mediastinal infection, guided by the time of development of the piercing event and its adequate evaluation and management in a specialized center. OBJECTIVE: We aim to record the occurrence of piercing cervical trauma by a foreign body – chicken bone fragment – with lesion of the larynx and hypopharynx giving rise to cervico-mediastinal emphysema in danger space, its anatomical-clinical correlations, early approach and resolutive conduct in a specialized center. CASE REPORT: Individual of 53 years, male, with foreign body swallowing about 2 hours of medical evaluation in specialized otorhinolaryngology emergency room. Presenting a perforating wound in the anterior cervical region, the foreign body lodged mostly in the right piriform sinus, with a portion piercing the posterior wall of the hypopharynx, continuing the pertuito to the danger space. There is no similar case reported in the medical literature. The radiological documentation is presented in this article. Despite the potential severity of the injury, the patient evolved well after approach and did not require a surgical approach after the removal of the foreign body by rigid direct laryngopharyngeal video. CONCLUSION: The case in question documents through imaging examinations the complex anatomy of the retropharyngeal space and deep cervicals, demonstrating the possibility of a minimally invasive approach in traumatic lesions of the larynx and hypopharynx, upon adequate evaluation by a specialist and resolute conduct having the accessibility of imaging exams and laryngopharyngealscopic video intervention. Keywords: Piercing foreign body ; cervical foreign body ; retropharyngeal foreign body ; foreign body in danger space.
