Hemobilia: A Rare Cause of Upper Gastrointestinal Bleeding

Hemobilia, a rare and potentially life-threatening cause of upper gastrointestinal bleeding, is often precipitated by hepatobiliary trauma or invasive procedures. We present the case of a 43-year-old male with ethanol related decompensated chronic liver disease, who developed severe hemobilia following ERCP and CBD stenting for choledocholithiasis. The patient experienced significant hemorrhagic symptoms, including melena and a rapid decline in hemoglobin, which were ultimately attributed to a hepatic artery pseudoaneurysm. Timely intervention with digital subtraction angiography and superselective arterial embolization resulted in successful management, highlighting the critical role of early diagnosis and interventional radiology in the treatment of severe hemobilia.
