Appropriate choice of intraocular lenses in present-day conditions: expert opinion

Appropriate choice of an artificial eye lens is an important factor in patient satisfaction with a surgical outcome. Currently, there are a significant number of different models of intraocular lenses (IOL). The optical eye system has age-related aberrations decreasing quality of vision (including those reported under mesopic conditions). So far, all IOL models can be divided into spherical, aspherical, aberration-neutral and aspherical, aberration correcting ones. Depending on the type of optics, monofocal, EDOF and full-range (multifocal) IOLs can be singled out. Spherical monofocal IOLs are more common in elderly patients with a rigid pupil and moderate postoperative expectations, as well as in persons with severe weakness of zonules of Zinn, significant abnormalities of the retina and optic nerve. EDOF IOLs can be recommended as premium ones for dysfunctional eyes (e. g., glaucoma optic neuropathy, age-related macular degeneration, etc.) In addition, such implants should be considered in patients with a single seeing eye and high postoperative expectations. Photopic effects and reduced contrast sensitivity are main disadvantages of multifocal IOLs. Aberration-neutral lenses are the most suitable for post-refractive surgery patients whose cornea has significantly different parameters.
