Annual Academic Meeting 2025 Abstracts
Introduction: Risk-reducing hysterectomy (RRH) is the most effective endometrial cancer (EC) preventive strategy and is recommended for women with Lynch-Syndrome.There is no data on health-related-quality-of-life after RRH, although this is critically important for counselling and economic evaluations. This research aims to evaluate quality-of-life, develop health-relatedutility-scores (HRUS) following RRH, and determine the lifetime EC-risk at which RRH and the levonorgestrel-releasingintrauterine-system (IUS) are cost-effective.Methods: The multicentre PRESCORES (ISRCTN17432105) study uses EQ-5D to derive HRUS for RRH using two methodologies: a cross-sectional study of women with Lynch-Syndrome (part-1), and a randomised vignette study of premenopausal RRH at 4 and 12 months (part-2).Part-3 uses these HRUS in a Markov cost-utility analysis to compare RRH and the IUS against no prevention at varying levels of lifetime EC-risk, with sensitivity and scenario analyses including for obesity. Results: 733 patients recruited to PRESCORES were included in this analysis for part-1; 373 following RRH, 360 controls.Women < 51 years following RRH had a -0.04 [95% CI: -0.08, -0.01] lower adjusted HRUS than premenopausal controls (p < 0.001).There was no difference in HRUS in women 51 years following RRH vs. postmenopausal controls (p = 0.18), nor in overall RRH vs. control groups (p = 0.67).Women with Lynch-Syndrome had
