A Case Series of Septicemia Due to Community-acquired Panton–Valentine Toxin Positive Methicillin-resistant Staphylococcus aureus in Children in Abidjan, Côte d\'Ivoire

Abstract

In this case series, we describe the epidemiological, clinical, and therapeutic aspects of 12 cases of CA MRSA-PVL leading to sepsis in children. IntroductIonStaphylococcus aureus (SA) infections are one of the leading causes of morbidity and mortality worldwide. 1SA is a commensal bacterium of the skin and nose responsible for suppurative dermatoses, fasciitis, osteomyelitis, pneumonia, and toxin-mediated diseases. 1 S. aureus strains are capable of producing various enzymes, but <5% produce Panton-Valentine leucocidin (PVL), which is known to be a virulence factor. 2 Studies have shown that its toxic effect on leukocytes results from the synergistic action of two distinct exoproteins, namely LukS-PV and LukF-PV. 2 The management of staphylococcal infections has become more challenging due to the emergence of both hospital (HA) and community-acquired (CA) methicillin-resistant Staphylococcus aureus (MRSA) in recent decades. 1 In fact, recently, the World Health Organization (WHO) included MRSA prevalence as one of the indicators of antimicrobial resistance in the Sustainable Development Goals related to health target 3.d. 3herefore, staphylococcal septicemia is of great concern in pediatrics because of its multitude of presentations and growing resistance to antibiotics. 4,5Its increasing prevalence leads to an increase in antibiotic use and healthcare costs. 6The emergence of CA strains of Panton-Valentine leucocidin-positive MRSA (MRSA-PVL) constitutes a real threat for emerging countries such as Cte d'Ivoire.
