A narrow QRS tachycardia with conflicting findings: A current pitfall of diagnostic criteria?
A 13-year-old man without structural heart disease was referred to our institution for catheter ablation of a narrow QRS tachycardia. A 12-lead electrocardiogram (ECG) recorded during sinus rhythm showed no ventricular pre-excitation. Multielectrode catheters were placed in the high right atrium (HRA), His-bundle (HB) region, coronary sinus (CS), and right ventricular (RV) apex. Programmed RV stimulation revealed decremental retrograde conduction, characterized by a smooth conduction curve of stimulus-to-atrial intervals, with the earliest atrial activation observed at the CS ostium.
