PHARMACOKINETIC AND PHARMACOGENETIC FEATURES OF BEDAQUILINE IN MANAGING PULMONARY TB PATIENTS
The review analyses research data on pharmacokinetic and pharmacogenetic features of bedaquiline (Bq) metabolism in pulmonary TB patients. The main metabolic pathway of Bq involves CYP3A4; however, different polymorphic variants of CYP3A4 may either decrease the serum concentration of Bq or increase it. Some data witness that African Americans have increased Bq metabolism, which is confirmed by poor TB treatment outcomes in this population. Researches into genetic polymorphisms in CYP3A4 have been initiated in the Russian Federation. Pharmacogenetic and pharmacokinetic features of Bq and other TB drugs represent a promising research area aimed to improve effectiveness and safety of management of drug-resistant TB.
